| Disease | TWO GROUPS OF CHILDREN WERE FOLLOWED PROSPECTIVELY FOR A MINIMUM OF 3 YEARS AFTER CYSTOPLASTY. GROUP 1 CONSISTED OF 30 CHILDREN WHO WERE CATHETERIZING THEIR NATIVE URETHRA AND IN WHOM ILEOCYSTOPLASTY HAD BEEN UNDERTAKEN AS A BLADDER AUGMENTATION FOR BLADDER NEUROPATHY. GROUP 2 CONSISTED OF 30 CHILDREN WHO WERE CATHETERIZIN BY A MITROFANOFF CHANNEL AND WHO HAD A CONTINENT CUTANEOUS DIVERSION ASSOCIATED WITH AN ILEOCYSTOPLASTY. THE UNDERLYING PATHOLOGY WAS A NEUROPAHTIC BLADDER IN 17 CHILDREN AND BLADDER EXSTROPHY/URETHRAL VALVES IN 23. ALL CHILDREN WERE INSTRUCTED TO PERFORM BLADDER IRRIGATION WEEKLY AND ENCOURAGED TO DRINK AT LEAST ONE GLASS OF CRANBERRY JUICE EACH DAY. ALL WERE INSTRUCTED TO TAKE CHEMOPROPHYLAXIS WITH LOW-DOSE TRIMETHOPRIM. TWO PATIENTS IN GROUP 1 FORMED STONES, FIVE PATIENTS IN GROUP 2 FORMED STONES. MEAN TIME TO STONE FORMATION WAS 36 MONTHS. WHILE INCIDENCE OF STONES WAS LOWER THAN IN OTHER PEDIATRIC SERIES, THERE IS NO FIRM EVIDENCE THAT WEEKLY IRRIGATION AND OTHER TREATMENT SIGNIFICANTLY ALTERED THE INCIDENCE OF BLADDER STONES IN CHILDREN. HOWEVER, MEAN TIME TO FORMATION OF STONES WAS A YEAR LONGER THAN IN OTHER SERIES. |
